What we collect about you, why we collect it, who we share it with, and what you can ask us to do with it. Your dental record is one of the most personal things you will ever hand over, and this page sets out how we look after it.

PRIVACY POLICY

YOUR RECORD IS YOURS.

Health information is treated as sensitive information under Australian privacy law, and we are held to a higher standard because of it.

Dental records hold your medical history, your medications, and photographs of your face. Australian privacy law treats health information as sensitive information and holds us to a higher standard because of it.

We collect what we need to treat you safely, we share it only where your care or the law requires, and we do not sell it. You can ask for a copy of your record at any time, and you do not need to give us a reason.

WHO WE ARE.

Adore Dental is a trading name of Sky Health Pty Ltd (ABN 89 663 581 342), a dental practice at Lot 26/91-95 Linton Street, Kangaroo Point QLD 4169.

We are bound by the Privacy Act 1988 (Cth) and the thirteen Australian Privacy Principles. As a health service provider we are covered by that Act regardless of our size: the small business exemption does not apply to us. We also handle clinical records in line with our obligations under the Health Practitioner Regulation National Law and the Dental Board of Australia’s requirements.

This policy covers information collected at the practice, over the phone, by email or SMS, through adoredental.au, and through our booking and enquiry forms.

WHAT WE COLLECT.

Identity and contact details

  • Your name, date of birth, address, phone number, email address, and an emergency contact where you give us one.

Health information

  • Your medical history, medications, allergies, dental history, clinical notes, treatment plans, X-rays, scans and clinical photographs.

Payment and billing information

  • Medicare or DVA details, private health fund and membership details, payment plan applications, invoices and receipts. Card payments are processed by our payment provider and we do not store full card numbers.

Correspondence

  • Enquiries, appointment reminders, recall notices and any messages between us.

Website information

  • Pages viewed, device and browser type, approximate location derived from your IP address, and cookie identifiers.

Health information is sensitive information under the Privacy Act. We collect it with your consent, and we collect only what is reasonably necessary to provide your care.

Where you book or attend with a child, we collect their information from you as their parent or guardian.

HOW WE COLLECT IT.

Directly from you wherever we can: on your new patient form, in conversation at your appointment, over the phone, by email, or through a form on our website.

From other people where it is relevant to your care and you have agreed to it: a referring dentist or doctor, a specialist, another practice transferring your records, Medicare or your health fund, or a parent or guardian.

Automatically through our website, using the cookies and analytics tools described in section 008.

You are welcome to deal with us anonymously or under a pseudonym for a general enquiry. We cannot provide dental treatment on that basis, because we cannot treat you safely without knowing your identity and your medical history.

WHY WE COLLECT IT.

  • To assess, plan and provide your dental treatment safely
  • To book appointments and send reminders, recalls and follow-ups
  • To quote, invoice and take payment, and to claim from Medicare, DVA or your health fund
  • To arrange referrals, laboratory work and anaesthetic services
  • To meet our record-keeping, professional and legal obligations
  • To respond to enquiries, resolve complaints, train our team and improve the practice
  • To send you marketing, where you have agreed to receive it

 

We will not use your health information for a purpose you would not reasonably expect without asking you first.

WHO WE SHARE IT WITH.

Other treating practitioners

  • Specialists, your GP, or another dentist, where we refer you or where your care requires it.

Dental laboratories

  • Prescriptions, scans, shade records and photographs needed to make your restorations.

Sleep Anaesthesia

  • Where twilight sedation or general anaesthetic is arranged, this independent team attends our clinic. They assess you, take your consent, keep their own records and bill you separately. Their privacy policy applies to the information they hold. Local anaesthetic and happy gas are provided by us and involve no third party.

Medicare, DVA and private health funds

  • For claiming, rebates and eligibility checks.

Payment and payment plan providers

  • Only the information needed to assess and administer an arrangement you have applied for.

Our software providers

  • Practice management, booking, email and SMS systems that store information on our behalf under contract.

Professional advisers and insurers

  • Our lawyers, accountants and indemnity insurer where necessary, and debt recovery services for unpaid accounts.

Regulators, courts and government agencies

  • Where we are required or authorised by law to disclose.

We do not sell your information, and we do not share your health information with advertisers.

Overseas storage. Some of the software we use stores information on servers outside Australia, including in the United States. Where that is the case we take reasonable steps to ensure the provider handles your information in a way consistent with the Australian Privacy Principles.

PHOTOGRAPHS, VIDEO & SOCIAL MEDIA.

Clinical photographs and scans are taken as part of your care and form part of your record.

We only use an image of you publicly, on our website, on social media or in advertising, with your separate written consent. Agreeing to clinical photographs for your record is not the same as agreeing to publication, and we will not treat it that way.

Before we use an image we will tell you where it will appear. You can withdraw your consent at any time by contacting us, and we will stop using the image from that point. We cannot always retrieve copies that other people have already downloaded or shared.

Where we film or photograph inside the practice for our own marketing, we will ask you first or arrange it so that you are not captured.

MARKETING, & HOW TO OPT OUT.

Where you have agreed to it, we may send you emails or SMS about the practice, our price guide, new services or offers.

Every marketing email has an unsubscribe link. You can reply STOP to any marketing SMS. You can also email or call us and ask to be taken off the list, and we will action it.

Opting out of marketing does not stop appointment reminders, recall notices, or messages about your treatment or account. Those are part of your care.

We do not use your health information to target advertising to you.

OUR WEBSITE, COOKIES AND ANALYTICS.

adoredental.au uses cookies and similar technologies so the site works properly and so we can measure how it is used. This includes Google Analytics and Google Tag Manager, and advertising tools including the Meta pixel.

These tools may collect your device and browser details, the pages you view, approximate location from your IP address, and identifiers that allow our advertising to be shown to you on other websites and platforms.

You can block or delete cookies in your browser settings. Some parts of the site may not work as intended if you do.

Our website links to third parties, including our online booking system, our payment providers and our social media accounts. Once you leave our site, their own privacy policies apply.

HOW WE STORE & PROTECT IT.

Your records are held electronically in our practice management system, with any paper documents stored securely on site.

Access is password protected and restricted to team members who need it to do their job. Everyone at the practice is bound by confidentiality obligations, and our clinicians are bound by their professional obligations as well.

We keep backups, and we take reasonable steps to protect your information from misuse, interference, loss, and unauthorised access, modification or disclosure.

If a data breach happens that is likely to result in serious harm to you, we will notify you and the Office of the Australian Information Commissioner as required under the Notifiable Data Breaches scheme.

HOW LONG WE KEEP IT.

We keep adult dental records for at least seven years from the date of the last entry.

Where you were under 18 at the date of the last entry, we keep the record until you turn 25.

We may keep information longer where a law, an insurer, or an unresolved complaint or claim requires it. When we no longer need information and are not required to keep it, we destroy or de-identify it securely.

ACCESSING & CORRECTING YOUR RECORDS.

You can ask for a copy of your record, including your X-rays and scans. Put your request in writing to [email protected]. We will ask you to confirm your identity, and we will respond within 30 days.

An administrative fee may apply for copying or transferring records. We will tell you the amount before we do the work. We will not withhold access because of an outstanding account.

In limited circumstances we may refuse access, for example where giving it would pose a serious threat to the life, health or safety of any person, or would unreasonably affect someone else’s privacy. If we refuse, we will tell you why in writing and how you can have that decision reviewed.

If something in your record is wrong or out of date, tell us and we will correct it. Clinical notes are not deleted, because the record needs to show what was known at the time. Where you disagree with a clinical entry, we will add a note recording your view alongside the original.

COMPLAINTS.

Tell us first if you can. Email [email protected] or call 07 3532 8604 and ask for the clinic manager. We will acknowledge your complaint within five business days and respond in writing within 30 days.

You do not have to come to us first, and you can go to either of the following at any point:

CHANGES TO THIS POLICY.

We update this policy from time to time as our systems and obligations change. The current version and the date it was last updated appear at the top of this page. Where a change is significant, we will say so here.
